now accepting funding applications for programs matching its

Types of Funding and How to Apply
1. Project and Capital Support
This category provides funding for project, capital, and one-time
expenses tied to Verdant Health Commission's priorities. Applicable
uses of funds include events, expenses for one-time or short-term needs
in the community, equipment and capital investments.

2. Multi-Year Health Program Support This category provides funding to support and expand health and wellness programs in our district. Programs will be selected via a competitive process and are renewable based on performance and results.
-To apply for funding, please see the Multi-Year Program Application (P200A)(Word document) and Attachments (P200B) (Excel). Applications should be submitted via electronic mail to info@verdanthealth.org.
3. Community Health & Wellness System Improvements This category supports innovative initiatives that improve health systems, neighborhoods, organizations, and networks. The goal of these initiatives is to address underlying community health issues with a focus on solving root causes of problems.
-If you are interested in exploring a partnership in this area, please contact Commission for Health staff or submit a brief letter of interest to info@verdanthealth.org describing your project/program.
Only complete applications will be considered for funding. If you are not sure about your eligibility or would like to discuss which category to apply under, you may submit an inquiry to info@verdanthealth.org or contact George Kosovich, Director of Programs & Community Investments at 425-582-8572. Applications are reviewed on a rolling basis.

Programs must:- Show a strong connection to the Commission's mission and priorities
- Define potential measurable results
- Demonstrate a clear connection between programs activities and results
- Primarily serve Verdant Health Commission / Public Hospital District No. 2, Snohomish County residents. View the district map to verify our area served.
- Leverage other resources
- Are evidence-based
- Use innovative strategies
- Are sustainable
- Partner with others in the community
